This form contains 7 sections:

Inspection
- Have responsible staff for the system experienced any problems with the system?
Camera Service Section
- Check physical condition of cameras and housings for signs of deterioration due to rain, dust and dirt.
- Check that field of view is correct.
- Examine supporting brackets and posts for signs of corrosion and damage.
- Check that all camera bracket fittings and clamping bolts are tight.
- Check that lenses are correctly focused.
- Check operation of auto-iris lenses as appropriate.
- Clean housing windows and dome covers as necessary.
- Check operation, blade condition and parking position of wiper unit, adjusting stop position if necessary.
- Check condition of pan / tilt unit, adjusting position of end stops and pre-sets where applicable.
- Check operation of infrared units.
DVR/NVR Service Section
- Check physical condition and cable input/output connections.
- Check environment for adequate ventilation for cooling fans
- Check HDDs for faults or bad sectors
- Check recording functions OK
- Check time & date stamp correct, adjust for daylight savings or apply NTP
- Test video export to USB and or video export to software
- Check operation of PAN/TILT/ZOOM control keyboard and connections.
+ ADVANTAGE Clients
- Do DVR/NVR/IPC cameras require firmware update
- Does software need updating. If yes list which devices have been updated - include previous firmware version and new firmware version updated to.
- Have new staff been trained in the operation of system, or existing staff had a refresher on same.If yes note Staff included and brief description.
