- Safety glasses/goggles/weld mask.
- Gloves worn, right for job?
- Respiratory and hearing protection.
- Safety harness, attachment to std.
- Fall protection.
- Finger rings not worn.
Work Activity Controls
- JSA in place/understood/relevant.
- LO/TO checklist in place, tags on.
- Spotters & watch persons in place.
- Weather/environment protection.
- Lighting.
- Scaffolds to std & scaffold tag.
- Access/egress to scaffolds, excavations.
- Hygiene monitoring required eg fibres, co monitoring in excavations and ventilation.
- Barricades & tarps managed.
- Sloping/shoring/benching.
- Permits in place, hot work, excavation, tie in.
- Form work/star pickets reo-bar capped.
- Gluts & equipment stored.
- Trip hazards managed.
- Chemical management, availability of MSDS.
- Grinder equipment inc disk speed and guard.
- Welding, cutting equipment & screens.
- Fire protection/prevention, sparks contained.
- Rubbish scrap and refuse management.
- Electrical leads elevated, tagged, ELCB'S used.
- Overhead work controls.
Equipment Focus
- Log books in place in cranes, EWP'S ect.
- Excavators/ earth movers condition.
- Compressors air receivers.
- Air hose secured, clips not in walkways.
- Correct pressure rating for hoses.
- Welding machines.
- Pipe stand inspection.
- Oxy acetylene/ flash back arrestors.
- Tirfors comalongs etc.
- Compactors.
- Concrete pumps/trucks.
- Rigging gear(slings, chain blocks) condition.
- Cranes, EWP'S and winches.
- Exclusion zones mobile equipment HPW etc.
