What does this form include?
This form contains 7 sections:
Outdoor Area
- Garbage is properly stored to avoid pests and insects
- Property kept clear of litter, combustibles, hazardous materials and chemicals
- Walls, floors, and ceiling free of possible penetration to avoid pests, dusts, smoke etc.
- Outside lights in good working order
- Exits and entrances are unobstructed and free of debris or storage
- Rooftop access is restricted, locked and sign posted
Pathways
- Entry mats are free of slip and trip hazards
- Evacuation plan/map is up to date and is posted in pathways (lobbies, exit, entrance etc)
- Walkways inside the site are clear and free of obstruction
- Floors inside the site free of cracks, slip and trip hazards
- Warning sign available for use if floor is wet and slippery
- Stairways adequately lit
- Stairways free of obstruction and is not slippery
- Stair treads, handrails in good condition
- Warning sign available for use if there are fall hazards in walkways (floor and stairs)
- Exit signs illuminated and is visible
- Exit routes kept clear and free of obstruction
Work Area
- Air quality adequate
- Temperature and humidity adequate
- Adequate lighting on work areas and pathways present
- Work areas free from glare
- Aisles, doorways and corners free of obstructions and permits visibility and movement
- Chairs in safe condition and are caster; rungs and legs sturdy
- Ceiling tiles intact, undamaged and in place
- All windows unbroken and free from any type of damage
- Air conditioning vents and ducts appear to be clean upon visual inspection
- All equipment and supplies are in their proper places
Common Areas
- Washrooms are clean; free of slip and trip hazards
- Food preparation area is clean; free of slip and trip hazards
Emergency Equipment & Procedures
- Entry and exit procedures provide workers personal security
- Emergency (evacuation, fire, bomb threat, hostile person) procedures in place
- Path to first aid kit clear of obstruction
- Fire extinguishers mounted, accessible, free of obstruction
- Fire extinguishers inspected internally monthly
- Security lighting installed and is in good condition
