What does this form include?
This form contains 4 sections:
1. OH&S Risks identified and addressed.
- 1. OH&S Risks identified and addressed.
- 2. Environmental Risks Identified.
- 3. Emergency contacts known.
- 4. Dial Before You Dig (DYBD) Plans on site.
- 5. Employees inducted & competent (VPL ID).
- 6. Dedicated waste / disposal area allocated.
- 7. Environmental controls have been forwarded to the field.
- 8. Guards / Barriers / Signs positioned in correct locations.
- 9. Traffic / Pedestrian Management in place.
- 10. Traffic Management Plan completed.
- 11. Temporary reinstatement completed.
- 12. Removal of Guards / Barriers / Signs.
- 13. JSA & SWMS completed.
- 14. Practical Completion documents complete and accurate.
6.1 Splicing Type 2 Architecture (LJL-DJL-TJL).
- 1. Manhole/Pit location as per design or approved variation.
- 2. Location of joint TJL - DJL - LJL hazardous.
- 3. Correct cable length within pit/manhole.
- 4. Correct housing of cable within pit/manhole.
- 5. Enclosure supported within pit/manhole.
- 6. Enclosure labelled correctly.
- 7. Cables labelled correctly.
- 8. Correct port allocation & usage as per design.
- 9. Heat shrinks (no more than 10mm slippage and sufficient heat applied).
- 10. Correct Cable sizes.
- 11. Sealing ring fitted.
- 12. Cables secured correctly within the enclosure.
- 13. Cable sheathed stripped to specification.
- 14. Frame assembly mounted correctly.
- 15. Correct buffer tube lengths.
- 16. Ribbons thoroughly cleaned & free of gel.
- 17. Ribbons neatly & correctly routed into storage areas.
- 18. Minimum 2 loops of slack in storage area.
- 19. Storage tray covers installed.
- 20. Ribbons neatly & correctly routed into splicing positions. Spliced correctly.
- 21. Ribbon correctly labelled.
- 22. Unspliced ribbons labelled and stored in correct position.
- 23. Splice tray cover correctly labelled.
- 24. Enclosure clean a free of foreign matter - water etc.
- 25. Enclosure have no missing internal parts / tray cover.
- 26. Clean and abrade all ports and cables.
- 27. Confirm pressure test has been conducted.
- 28. Site left clean and tidy.
- 29. Site left safe.
- 30. Checked with "As Built" Data spliced to Design.
- 31. Installation meets all other design documentation, specifications and approved variations.
6.2 Splicing Type 2 Architecture (AJL).
- 1. Manhole/Pit location as per design or approved variation.
- 2. Location of joint AJL hazardous.
- 3. Correct cable length within pit/manhole.
- 4. Correct housing of cable within pit/manhole.
- 5. Enclosure supported within pit/manhole.
- 6. Enclosure labelled correctly.
- 7. Cables labelled correctly.
- 8. Correct port allocation & usage as per design.

7.1 Fibre Testing (LN/DN & FDH GPON Splitter).
- 1. Test equipment calibrated.
- 2. Insertion Loss tested with a Light Source & Power Meter. OTDR Bi-Directional. 1310, 1625nm wavelengths. Optical Loss shall not exceed calculated optical budget.
- 3. Test compiled in accordance with the specification and Test results recorded - Look for any reds in workbook.
- 4. If Insertion loss exceed optical budget, check whether sub-contractor has filled the fibre fault trouble shooting report and they have replaced the faulty part.
