What does this form include?
This form contains 3 sections:

STRUCTURE
- Caster wheels move freely.
- Mast, legs, and base are not bent or cracked.
- Raise mast sections and inspect for free, smooth sliding action.
- Is the capacity decal/plate present and readable?
RIGGING
- Inspect the cable for kinks and frays. If kinked, or more than three wire strands are broken (small wires) do not use the lift until the cable has been replaced.
- Winch operates freely and cable is not tangled on the winch drum.
- Winch handles are in good repair and not bent. Winch covers are in place.
- Pulley housings are not cracked and are free of indentations which can restrict the rotation of the pulleys.
- Cable is seated in all pulleys.
- Rollers rotate freely without binding.
