This template is used to conduct onsite qhse inspections.
What does this form include?
This form contains 22 sections:
General Site Access Conditions
- Chain lock system operational & maintained
- Barricades/cones/tape/temp. fences in place to prevent public access
- Site maintained (e.g. No long grass)
- Structurally sound
- Safety & warning signs in place
- PPE signs in place
- Site clean
- Overhead work area secured from public access
- Access ways clear of debris & tools?
- Adequate lighting/ventilation
Inductions and Access to Site
- Site visitors inducted and record with signature available on SWMS and/or Visitor Logbook
- Personnel OHS induction cards available on site
- Personnel tickets available on site for the task being undertaken
Management System
- Emergency/Rescue procedure in place
- SWMS adequate and reviewed on site by workers. Subcontractor SWMS To be verified against the GRA SWMS Checklist (GRASFM24)
- Pre-Start meeting and Toolbox Talk records available
- Permits/Approvals to work obtained (ie. Hot Works, Confined Space, CR’s)
- Safety inspections (daily/weekly) and assessment checklist undertaken for high risks works (Plant, W@H, excavation, etc)
- People verified competent to perform tasks
- Maintenance records / logbooks available (I.e. rigging gear)
- Adequate Supervision for all the tasks performed
- GRA Project Safety Management Plan available for consultation (office filing system)
Work At Height
- Is there Any Work at Height?
Elevated Work Platform - EWP
Crane and Other Mobile Plant
- Is there a Crane or other mobile plant on site?
Traffic Management
- Parking adequate
- Vehicles separated from people
- Delivery/laydown area defined as required
- Traffic Management in place where required and plan available on site
Personal Amenities
- Toilet facilities identified locally
- Water available on site
- Rest area identified
First Aid and Emergency
- First aid kits readily accessible and fully stocked
- Fire extinguisher available at work site location (Tested & In date)
- Emergency response plan in place with local medical centre details, first aid kit and fire extinguisher locations defined
- Assembly area defined
- Emergency contact details displayed / available
- 2 Senior first aiders available on site
Personal Protective Equipment - PPE
- Are all persons wearing company issued long sleeve shirts & full length trousers
- Hardhats worn on site (In date)
- Safety footwear appropriate
- Respirators available as required
- Safety glasses worn as required
- Hearing protection available for task
- Double eye protection (face shield or goggles available) for high impact tasks
- Sun protection available
- Radiation detection equipment available (calibrated)
- Gloves available as required
Manual Handling
- Safe work practices adopted
- Correct posture used for task
- Correct tools and lifting devices used for task
- Handles fitted where required
- Weight indicated where possible
Tools and Equipment
- Fit for use, no damage
- Guards fitted
- Stored correctly
- Lifting Equipment tagged and tested
- Safe working loads I weight indicated

- Calibration certificates available for test equipment
Electrical
- Power Supply Adequate
- Tools & Equipment tagged and tested
- RCD's in use
- Leads on stands, protected from damage
Hot Works
- Are there any Hot Works scheduled on site?
Chemicals
- MSDS and Register readily available on site {soft or hard copy to be instantly readable during the use of chemical)
- Chemicals labelled correctly
- Storage adequate, bund where required
- Gas cylinders upright and secured
- Safe use of chemicals and adequate PPE worn
Compressed Air
- Is compressed air being used onsite?
Hydraulics
- Are there Hydraulics in use on site?
Dust, Noise and Fumes
- Identified or pre-defined roads are being used
- Dust suppression available, if needed
- Noise limited to an acceptable level
- No excessive odours or fumes
Water and Wet Areas
- Is there any excessive water or wet areas on site?
Excavations
- Are there any excavations planned?
Confined Spaces
- Is entry into confined space required?
Environment
- Air pollution - watering of sites to minimise dust generation
- Water Pollution- wastewater management to prevent discharged to storm water drains
- Noise Control - noise mitigation measures adopted
- Waste Management - Appropriate waste disposal, including recyclable waste areas/processes
- Waste Management - chemical waste, if any, collected and disposed of properly by licensed facility/personnel
- Protection of Flora - disturbance to flora minimised
- Protection of Fauna - disturbance to fauna minimised
- If cultural heritage aspects exist on site, preservation measures In place
