What does this form include?
This form contains 10 sections:

Housekeeping
- Is the building free from litter?
- Is refuse being picked up at frequent intervals?
- Do trash cute doors open and latch on their own?
- Are walls (including doors) covered with less than 20% for each section?
Fire Protection Equipment
- Are fire extinguishers located off the floor in a designated marked/labeled area and unobstructed?
- Are all sprinkler heads free from damage and paint?
- Does the sprinkler system have a current green tag?
- Does the sprinkler riser have a hydrostatic test plate on the riser?
- Are all sprinkler risers easily assessable and rooms free from storage?
- Are all sprinkler heads free from obstructions?
- Is the fire alarm panel cabinet locked?
- Is the alarm system normal? (Troubles, supervisories, and fire alarms)
- Are the FDC's and PIV's unblocked?
- Does the FDC has both connections covered?
- Is the PIV in the locked position?
General Fire Safety
- Is there a minimum of 36" clearance between storage and heating equipment?
- Are there any gasoline or natural gas powered equipment inside the building?
- Are there any flammable or combustible chemicals located outside of an approved storage container?
- Are all gas cylinders secure? Give specific locations in notes
- Are dryer vents free from lint and debris?
- Are any ceiling tiles broken or missing?
- Are any items hanging from the ceiling?
Exits and Doors
- Do all fire doors close completely and latch on their own?
- Are all exit and egress doors unobstructed (width of doors maintained throughout the egress path)?
- Can all exit doors open without any special knowledge, key, or unapproved device?
- Is there at least 10' of clearance outside all exit doors?
- Are there any visible penetrations in the fire walls that are not sealed with fire caulk?
- Are all stairways, stairwells, and corridors free from obstructions?
- Are all stairways, stairwells, and corridors free from storage of any kind?
Exit Signs and Emergency Lighting
- Are all exit signs lit (where lighted exit signs exist)?
- Are all exit signs in working order?
- Are all exit signs free from damage?
- Are all exit signs visible?
- Are all doors that could be confused as an exit marked "NOT AN EXIT"?
Electrical Safety
- Are there any extension cords being used? If Yes, are they being used for TEMPORARY purposes?
- Is there at least 36" of clearance between electrical service equipment (ie. Breaker panels) and storage?
- Are there any unapproved multi plug adapters being used? If so, list location(s) of adapters.
- Are all electrical outlets equipped with an approved cover?
- Are all spliced wires covered with an approved box?
Planning and Preparedness
- Are fire evacuation plans posted and visible?
- Are severe weather shelter plan posted and visible?
