This form contains 11 sections:

Optical Fibre Joints / Terminations
- Correct fibre count as per Design If not Designer notified
- Correct cable numbers and diameters in joint ports
- Ports used in correct sequence ie Main of for OJ used first
- Thermo shrink colour change OK
- No heat damage to cable sheaths / joint closures
- Correct foil installation no PE sheath exposed at closure/heat shrink entries
- Fibres free of grease
- Correct Colour Splice trays used and fibres housed correctly
- Correct splice protectors fitted
- Cable identification tags fitted
- Tags to be secured with TWO zip ties
- If tags are illegible or insecure, re-tag accordingly
- Enclosures pressure tested and pressure released (if required)
- Cable flow / loops installed correctly (no interweaving, MBR OK)
- Cable and joints racked / secured – stored cable length to customer specifications
- Appropriate glue block or branch clip used
- Cable entries sealed - building and exchange terminations
- Termination modules secured correctly
- Correct number and type of connectors fitted to terminations as per design
- Pigtails and patch cords not exceeding minimum bend radius or over tensioned
- Manholes/pits & work areas left clean and tidy (All waste disposed of correctly)
- If space available, lay up spare fibre in storage tray
- Amended plans marked up for return to Design Office
- Pride tag sticker on Joint /FAP
Optical Fibre Testing
- WORKBOOK results sent to Co-ordinator or inputted into WAMs
- Optical test instruments within calibration dates
- Optical connectors cleaned
- OTDR tests to specification
- All events marked and OTDR traces stored
- Link losses measured,recorded and to spec
- Splice losses measured and within spec
- Insertion loss recorded and Insertion loss minus link loss within spec
- All faults and abnormalities repaired/identified, located and recorded to spec
