What does this form include?
This form contains 5 sections:

Audit
- 1 . Cables formed correctly -
- 2 . Cables have minimum coil length - 15 meters preferred
- 3 . Cable labels on every cable ( FO number if provided ,core count , location )
- 4 . Heat shrink ( shrank properly, straight, green paint gone ,foils installed correctly)
- 5 . Strength member installed correctly ( no tubes behind strength member etc. )
- 6 . Split pin fitted correctly if Express joint(oval port)
- 7 . Biscuits installed correctly in tube holders
- 8 . Tubes allocated in tube holders correctly ( as per colour code )
- 9 . Buffer tube lengths in tube holder cut to correct lengths (staggered )
- 10 . Trays labelled correctly ( including numbering on side of trays )
- 11 . All fibres clean ( no grease or dirt on active or dark fibres )
- 12 . Dark fibres in splice trays labelled and laced up on top of active fibres ( Or in dark fibre storage )
- 13 . Minimum of 3 wraps of fibre to be spliced in each tray
- 14 . Correct splice protectors used ( size ,heat shrunk properly , installed correctly )
- 15 . Rubber O ring has been fitted and joint seal clean.
- 16 . Desiccant pack in joint ( moisture bag )
- 17 . Flash tested joint ( max pressure 5 kpa )
