- Does the supervisor act professionally?
- Is the information clear and relevant?
- Does the supervisor give opportunity for crew discussion?
- Is the meeting conducted in an timely and orderly fashion?
CREW SAFETY CARDS (WE CARE CARD):
- Has the supervisor written clear, specific instructions?
- Has the supervisor indicated any special precautions?
- Does the supervisor display a high level of competency in regards to how the card is to be properly used?
- Does the supervisors direct reports understand how they are to use the safety card?
- Does the supervisor utilize the safety card to engage the worker in the workplace?
- Does the supervisor follow up on the safety concerns of his workers?
HAZARD IDENTIFICATION
- Does the supervisor identify workplace hazards?
- Does the supervisor follow the "time to act" model when notifying workers of hazards?
HOUSEKEEPING
- Fuel/oil stored correctly?
- Explosives/caps stored correctly?
- Is the travel way and workplace clear of scrap?
- Is gear, resin and vent tubing stored correctly?
SERVICES
- Installed as per company standard?
- Vent tubing 50' from face and physically felt?
- Vent tubing in good condition?
- Vent board up to date?
- Air and water pipe installed as per company standards?
- Is pipe and vent installed where it does not pose a hazard in the travel way?
GROUND SUPPORT
- Installed according to print?
- Proper tools being used?
- Is the area well scaled?
- Proper PPE being used?
- Proper length and number of scaling bars present?
- Are workers trained and competent?
COMMUNICATION
- Does supervisor engage crews at "muck pile" talks?
- Give clear instructions?
- Holds crews accountable?
- Provide follow-up discussion?
- Provide quality cross-shift notes?
- Provides "captains log" information to communicate with other crews?
