What does this form include?
This form contains 15 sections:
Personal protective equipment
- Hard hats being used 100% of time?
- safety glasses being used 100% of time?
- Hand protection where needed?
- Hearing protection where needed?
- Arc Flash equipment where needed?
Ladders
- Proper height and type ladder being used?
- Are ladders in good shape? No splits, cracks, broken steps rungs or brackets
- Step ladders are fully open and spreaders locked
- No standing on top two steps
- Extension ladders extend 36" above landing and secure at top
Power And Hand Tools
- All tools in proper working condition
- All guards in place and working correctly
- Correct tool being used for the job
- Electric tools double insulated or grounded
- All personal hand tools are in good shape
General Electric
- Proper lighting provided in general construction areas
- Temporary lighting in good shape
- All G.F.C.I's are tested and functioning properly
- All panels have covers in place and are locked
Extension cords
- Cords are rated for hard or extra hard usage
- Ground prong is intact
- Cords have no cuts or abrasions
- Cords are 3 wire # 12 minimum
Panels
- Electrical panels are labeled appropriately
- Electric panel knockouts are in place
- Panel dead fronts and covers are in place
Temporary Lighting
- Light bulbs are protected from breakage
- Temporary lighting is 8' or higher off floor
- Temporary lighting is isolated from other objects
- Temporary lighting wire is not subject to pinch or crush points
Scaffolds
- Scaffold is set up correctly
- Guard rails and toe boards installed over 6'
- Ladder access to work level
- Scaffold is placed on sound, stable, rigid footing
- Employees on scaffold in trained and authorized
Aerial Lifts
- Is the lift in proper working condition, clean, and maintained
- Is lift being used on solid level ground
- Is the safety chain closed
- Full body harness being used if needed
- Is employee on lift aerial-lift trained
General job site safety concerns, housekeeping, and sanitation
- Overall condition of work area is acceptable
- Adequate trash removal
- Toilet facilities available and clean
- Floor openings covered and marked
- Employee trained in first aid/ CPR is on site
Office and storage trailers
- Clean, neat, and organized
- Stairs and rails in good shape
- Fire extinguishers available and properly charged
Safety procedures
- Employee have been trained in the proper procedures of lock out/ tag out
- Employee have been trained in the proper procedures of confined space
- Employee have been trained in the proper procedures of trench and excavation
- Employee have been trained in the proper procedures of fall protection
- Employee have been trained in the proper procedures of crane use
Equipment
- Employee operating the equipment are trained and authorized- forklift
- Employee operating the equipment are trained and authorized- backhoe
- Employee operating the equipment are trained and authorized- trencher
- Employee operating the equipment are trained and authorized- bobcat and/ or mini excavator
- All equipment is maintained and clean
