What does this form include?

Driver Details
- Drivers license class & expiry
- DG License expiry
- SSAN license expiry
- Delivery documentation complete and correct
- Is the driver wearing the appropriate PPE
- Orica passport (induction & training expiry)
- Does the driver understand the information displayed on the EIP
- Is the driver trained and familiar with the emergency response procedures
- Log books carried and completed ( when required)
Vehicle safety equipment
- Fire extinguishers carried as per DG requirements inside and outside cabin and adequately secured
- First aid kit in cab - products adequately supplied and within date
- eye wash kit in cab - products adequately supplied and within date
- Copy of ERG or EPG's for the products carried in drivers door
- Seat belts operational
- flashing light operational
- Reversing beeper operational
- Three reflective triangles
- Copy of SSAN driver security manual and emergency response procedure available
Prime Mover Standards
- Registration number
- Exhaust directed away from tank/trailer
- Battery firmly secured with electrically acid resistant cover
- Tyres in good condition & meet required standards (new tyres or retreads)
- Lights in working order
- Fuel tanks secured properly and in good condition, hoses and vents serviceable and clear
- Cabin clean and free of garbage and loose objects
Trailer Standards
- Registration Number
- Registration label/expiry
- Workcover SSAN license plate fitted
- Dangerous Goods EPA label license number and expiry
- Tyres in good condition and meet required standards (new tyres or retreads)
- Lights in good working order
- General condition of trailer/s
- Restraiing straps or chains in good condition. Ropes must not be used to secure loads
- Trailer floor must be metal, clean, free of combustible material and sharp edges
- Tarpaulin/ side curtains in good condition (if applicable)
- Twist locks in good condition
Load Restraint and Placarding
- Tauliners must have straps on each row inside curtains for bulk bags
- Slip sheet in place between trailer header board and front bulk bag, and between roof rails (for tautliners) and bags to prevent rubbing
- EIP's contain correct information and visible from both sides and rear of vehicle
- DG diamond fitted to front of vehicle, displayed clearly without obstruction
- Security seal's fitted properly as required for each trailer application
Additional tanker & Tank specific standards
- Tank Inspection plate current (Approval No., Test date, Build date and Test renewal due date)
- Camlock nozzle fitted with cover and in good condition
- No damage to pipe work, valves or valve handles and fittings protected from damage
- No signs of TACN on tank fittings
- Handrails and access ladders in good working condition. (Hand rails may be substituted via other fall risk prevention that must be approved each time by site supervisor)
- Confined Space warning signs on tanker inspection openings
