Work Environment
- Are the housekeeping standards satisfactory?
- Have interactions with other mining equipment and work groups been eliminated ?
- Is the machine or equipment in good condition?
- Is heat and hydration being managed?
- Is all equipment stored and secured safely?
- Is barricading, cones and signs in position?
- Are personnel responding correctly to spillage or floor conditions?
Systems and Standards
- Is the procedure or JSEA for this task understood and being followed?
- Are work permits for this task understood and being followed?
- Is the person trained and competent for the task?
If no HIT has been completed, work through the HIT process with the participants and details key hazards here
Tools, vehicles and equipment
- Has a pre start inspection been carried out on all equipment?
- Are all tools and equipment in good condition?
- Is all electrical equipment displaying the correct inspection tag, and within test date?
- Are vehicles and equipment being operated appropriately?
People and behaviours
- Is PPE being worn correctly?
- Is positive communication being used correctly?
- Is over exertion and poor positioning being managed?
- Are all workers using 3 points of contact where applicable?
- Can personnel explain the emergency procedures?
