What does this form include?
This form contains 7 sections:
1. Documentation
- JSEA reviewed and signed
- JSEA requirements adhered to
- ToolboxTalk file up to date/ Take 5
- Pre-Start Conducted and records kept
- Hazardous Substance SDS available
- Emergency Drills conducted as per schedule
- Training matrix on board and current
- Permit to work process (hot work, confined space, work at heights, man lift)
- Safety Management System Documentation on board
- Records of vessel inductions/visitor register
- Lifting register on board
- Handover process for masters in plans and effective
2. PPE
- Helmets, footwear, safety glasses, high vis long clothing worn
- Use of Lifejackets
3. Site Conditions
- Access and Egress on Vessels
- Barricading Appropriate - including information tags attached
- Signage Appropriate
- Housekeeping at Acceptable standard. Redundant material & rubbish material
- Excavations identified. Barricade/Windrows in place
- First Aid Kits available in vehicles and stocked
- Crib Hut & Ablutions clean/serviced
- Clear access to emergency services equipment
- Hazardous Substances used and stored as per MSDS
4. Tools & Equipment
- Electrical Equipment current tag, in good condition, used correctly &
- Rigging Equipment in good order, used correctly & safely
- Rigging Equipment current inspection tag/identification
- Equipment isolated as per procedure - Isolation tags and locks in used
- Mobile Equipment in good order - Vehicle/Plant Housekeeping
- Housekeeping of tools and equipment
5. Hot Work
- Gas cylinders stored upright & secured - Flashback arrestors
- Flammable material (rags, paper, timber, etc) removed from work area
- Sparks adequately encapsulated - Welding screens used where appropriate
6. General
- Check for visible leaks, hoses and couplings
- Personal Awareness of Environment aspects
- Supervisors active on safety
- Safe Work Methods
- Fire extinguisher tested and tagged
