Detailed inspection of service rigs
What does this form include?
This form contains 21 sections:
Service Rig #
General
- Adequate distance from overhead power lines?
- Is the service rig hazard sign displayed?
- Is the fall lane of the derrick clear?
- Are vehicles out of the fall lane?
- Is the overall housekeeping on the service rig satisfactory?
- Are catwalks satisfactory?
- Is the calibration of the fixed H2S rig monitor current?
- Are fire hazards controlled?
- Is the overall condition of the pad site satisfactory? (No trash or equipment scattered around)
Guying
- Are there wind guys?
- Are the condition of the guy lines acceptable?
- Is there a minimum of three wire rope clamps on each guy wire?
- Are the ground anchors in good condition and pull tested to 18,600 lbs. Is there a tag attached to the ground anchors?
Foundation
- Is adequate supplemental flooring available?
Hand Tools
- Are hand tools in good condition?
- Are hand tools clean condition?
- Are hand tools properly stored away?
Comments on tools
Mast & Derrick
- Is the manufacturers specifications plate on the Derrick?
- Is the mast/ derrick free of excess corrosion and damage?
Vehicles
- Are the vehicle glass and mirrors intact and free of cracks?
- Are the road gear, lights and reflectors intact and in working order?
- Is the cab free of clutter?
- Is the doghouse free of housekeeping issues?
- Are the lights in the doghouse free of hazards?
- Are the hitch and safety chains free of defects?
- Are flammable liquid storage cans satisfactory?
Carrier
- Are handrails in place on elevated walkways and steps?
- Are the tires and wheels in good working order? Are they free from cuts, gouges. Is the wheel showing any defects?
- Are the fuel tanks labeled?
- Is there any fuel leakage?
Drawworks Condition
- Are there any guarding hazards on the drawworks?
- Is the handling winch satisfactory,
Operator Station/Work Platform/Floor
- Are all operator controls labeled?
- Is the work floor free of hazards(i.e slip, trip and fall)?
- Are the steps and handrails in place?
Tools and Equipment
- Is the the safety gate on the tongs satisfactory?
- Is the stiff arm and snub line satisfactory?
- Are the head guards in good condition?
- Is the tong in good working order?
- Are the bales/links free of defects?
- Are the elevators operational and in good working order?
- Is there safety pressure relief valves?
- Is the discharge line from the relief valves anchored?
- Is there any machine guarding hazards with the pumps?
Safety Equipment
- Is there a safety harness and restraint lanyard provided at the tubing board, rod basket and stabbing board?
